*June 1st is NOT a deadline for REGISTERING for camp! Registration is still OPEN! June 1st is only a deadline for paying with a personal check. Certified checks and money orders (and school checks for Team Camp) are accepted anytime.
*If you register online, there is a one time processing fee PER registration charged by ACTIVE. If you register by mail, we can accept checks or money orders (no processing fee) until June 1st, but NO CASH is accepted at any time.
*From June 1st through the day of check-in, the ONLY payments we can accept are with a MONEY ORDER or certified check (no processing fee) or with a CREDIT CARD (which will incur the one time processing fee if you originally paid with a check or money order).
*Coaches who are coming to TEAM CAMP: a school check is accepted at anytime.

Medical Form Information
A copy of our medical form will be e-mailed following acceptance of registration and deposit. It can also be printed off from this page. The form MUST be completed and returned by the first day of camp. No camper will be allowed to participate in any camp until their medical form has been received. Forms should be filled out entirely and must include insurance information, a parent or guardian's signature, and either a doctor's signature OR an attached copy of a current school physical from 2009-2010 school year.
